Jay and Rose Phillips Aging Care & Connections (ACC) provides services and supports to help older adults remain as independent as possible for as long as possible in the environment of their choosing.
What We Do
The ACC team provides consistent care, eases transitions, and creates a vibrant support network for older adults and their families. We work to create a comprehensive care plan that meets each individual’s needs in a way that preserves dignity and maintains quality of life.
